Educational Consultant - Per Diem - $15.00 - $24.89/hour DOE

 



Medicaid Service Coordinator (MSC) - Full-time - $12.50 - $15.37/hour
  1. Maintains Service Coordination certification status.
  2. Provides objective service delivery with the individual served as the priority.
  3. Assists individuals with planning and decision making to achieve desired outcomes.
  4. Keeps abreast of community resources to optimize service delivery.
  5. Schedule and attend all Individual Service Plan meetings and other service related meetings, as needed.

     

  6. Ensures all services are documented and plans are completed to meet funding requirements, New York State, federal and agency regulations and/or requirements.
  7. In collaboration with appropriate sources, develops, signs, distributes and implements any and all paperwork relevant to service delivery. This includes Medicaid, SSI and DSS recertifications.
  8. Completes the necessary face-to-face contact with individuals served as required by New York State regulations.
  9. Remains knowledgeable and updates the ISP, SCA and CSA-P every 6 months, HIPAA and LCED once per year.
  10. Assists persons served with attending necessary appointments, and ensures that necessary paperwork is completed.
  11. Reports all incidents, illnesses or other unusual circumstances according to policies and procedures.
  12. Documents all significant service activities on Service Coordination case notes.
  13. Maintains a flexible and independent schedule in accordance with the desires and needs of the people served.
  14. Provides on-call emergency services to participants of the department as assigned. Performs other duties as assigned by supervisor.


Quality Assistance Liaison - $30,000 - $36,896

JOB RESPONSIBILITIES

 

 

  1. Actively develops reliable evaluation tools and investigation practices.

     

  2. Maintains accurate and up to date knowledge of VESID, OPWDD,
    United Way, New York
    State Education, OSHA and all other governmental codes, rules, laws and regulations that pertain to LDA services.

     

  3. Implements system of audits and oversight to all agency services and components of operation to assure ongoing assessment of efficient, sound process.

     

  4. Provides continuous feedback and appropriate recommendations to programs on auditing tools outcomes and practices. 

     

  5. Completes the Quality Assistance Outcome and Compliance Report.

     

  6. Functions as a member of the program planning team when requested.

     

  7. Acts as referral agent and advocate representing the interests of individuals receiving services.

     

  8. Completes OPWDD 147 investigative process.  Presents investigation findings for administrative review and the Incident Review Committee.

     

  9. Reviews and completes follow up (if appropriate) for Internal Events for assigned programs.

     

  10. Participates in, and provides education and training to, individuals who receive services and staff.

     

  11. Develops and conducts quality improvement surveys for individuals receiving services and other appropriate parties as requested.

     

  12. Acts as agency liaison for OPWDD and other community organizations.

     

  13. Performs other duties as assigned by supervisor.

     



Residential Habilitation Counselors - SUPERVISED IRA - FT, PT and Relief - $10.10 - $12.42/Hour

Implements individual Habilitation Plans and provides the necessary ongoing instruction and support services to maximize individuals’ abilities to live competently as integrated members of the community.  Instructs, supervises and assists individuals in skills based on their ISP’s, Habilitation Plans, and Individual Plans of Protection, which allow for greater strengths and independence in their lives.  These services should be person centered and focused on individualization, integration, independence and productivity.

1. Functions as a member of the interdisciplinary team in the formulation and the execution of Individualized Service Plans (ISP’s).
2. Implements individuals’ Residential Habilitation Plans and Individual Plans of Protection, and supervises and assists individuals in meeting the objectives in their plans.
3. Completes all required Residential Habilitation Service billing documentation in an accurate and timely manner.
4. Completes Client Contribution Forms on designated residents in a timely and accurate manner and assures that client contribution fees are collected and submitted to LDA during the pay period that they are owed. Notifies Residential Manager immediately if there are any problems.
5. Provides direct personal care and daily living skills when the person is unable to do so due to disability issues, illness, injury or behavioral challenges.
6. Responsible for the residential setting, grounds, furniture, equipment and relevant property being kept in a safe, clean, functional and sanitary condition at all times. Notifies Residential Manager immediately if a problem occurs that can not be corrected by Residential Habilitation staff.
7. Performs in compliance with OMRDD regulations and with all LDA policies and procedures and site specific program policies and procedures manuals.
8. Maintains participant records according to policies and procedures and as directed by supervisor.
9. Reports all incidents, illnesses or other unusual circumstances according to policies and procedures.
10. Supervises residents’ nutritional/dietary activities and needs.
11. Responsible for obtaining and maintaining First Aid, Medication Administration, CPR and other designated certifications and keeping updated.
12. Responsible for administering medications following AMAP guidelines and LDA policies and procedures, and for providing transportation and support when requested by Medical Liaison and/or Manager.
13. Assists residents in travel training to appointments and other functions.
14. Assists residents in organizing, coordinating and attending recreational/community activities.
15. Attends all required trainings and other meetings deemed necessary by supervisor.
16. Assures that all staffing requirements for the site are met at all times by working designated shifts unless PTO and/or UPTO is approved by supervisor. At times may be required to work additional hours to meet this requirement when requested by supervisor.
17. Performs other duties as assigned by supervisor.

QUALIFICATIONS
* Associate’s Degree with one year of hands-on experience working with individuals with developmental disabilities preferred, or High School Diploma or GED with one year of proven work history required. Prefer candidates who have worked with individuals with disabilities.
* Valid New York State driver’s license and an insured, registered motor vehicle.
* Must be ambulatory.
* Ability to work flexible hours.
* Upon completion of obtaining AMAP Medication Certification, all full time employees must maintain AMAP Medication Certification status.
