Description:
  1. Maintains Service Coordination certification status.
  2. Provides objective service delivery with the individual served as the priority.
  3. Assists individuals with planning and decision making to achieve desired outcomes.
  4. Keeps abreast of community resources to optimize service delivery.
  5. Schedule and attend all Individual Service Plan meetings and other service related meetings, as needed.

     

  6. Ensures all services are documented and plans are completed to meet funding requirements, New York State, federal and agency regulations and/or requirements.
  7. In collaboration with appropriate sources, develops, signs, distributes and implements any and all paperwork relevant to service delivery. This includes Medicaid, SSI and DSS recertifications.
  8. Completes the necessary face-to-face contact with individuals served as required by New York State regulations.
  9. Remains knowledgeable and updates the ISP, SCA and CSA-P every 6 months, HIPAA and LCED once per year.
  10. Assists persons served with attending necessary appointments, and ensures that necessary paperwork is completed.
  11. Reports all incidents, illnesses or other unusual circumstances according to policies and procedures.
  12. Documents all significant service activities on Service Coordination case notes.
  13. Maintains a flexible and independent schedule in accordance with the desires and needs of the people served.
  14. Provides on-call emergency services to participants of the department as assigned. Performs other duties as assigned by supervisor.

Qualifications:
  • Associates Degree in human service, or 20 human service credit hours earned as part of an Associates Degree is required.

     

  • Minimum of one year experience working with individuals with developmental disabilities OR as a Service Coordinator with any population, required. Experience requirement waived with Master’s Degree in health and human services field.

     

Requirements:

  • Within first six months of employment, must become New York State Certified as a Service Coordinator.
  • Annually must attend 15 hours of New York State mandated professional training.
  • Must have a valid New York State driver's license and a registered, insured vehicle.
  • Excellent written and verbal communication skills required.
  • Excellent customer service skills required.
  • Ability to independently manage and meet multiple deadlines required.
  • Good Microsoft Office skills desired
