Description:

Works as part of the residential team to assure that all medical appointments, medical related care and all medical documentation are addressed and completed according to regulatory standards and LDA policies and procedures.

1. Performs in compliance with all regulatory standards and agency and residential policies and procedures.
2. Functions as a member of the interdisciplinary team, under the direction of the Registered Nurse, in the formulation and the execution of the residents’ Individualized Service Plans (ISP’s) and Individual Plans of Protection (IPOP’s). Assures that staff are familiar with all information on the IPOP on an ongoing basis and prior to the residents’ six month reviews.
3. Assists residents in scheduling medical and therapy appointments based on the needs of each resident.
4. Acts as a liaison between physicians and internal/external parties regarding medical services.
5. Complete all required medical related documentation in an accurate and timely manner. Also must maintain medical progress notes.
6. Maintains medical section of participants’ records according to OMRDD regulations, LDA policies and procedures and as directed by supervisor.
7. Responsible for assuring that all medical appointments and follow up are documented in the medical appointment ledger.
8. Maintains medication administration records according to OMRDD regulation, LDA policies and procedures, and as directed by supervisor.
9. Responsible for administering medications following AMAP guidelines and LDA policies and procedures.
10. Responsible for maintaining a seven day supply of all medications, over the counter medications and medical supplies in the home at all times.
11. Responsible for monitoring residents’ prescribed medications and over the counter medications for expiration dates and the need to dispose of and replace.
12. Reports all incidents, illnesses or other unusual circumstances according to policies and procedures.
13. Provides direct personal care and daily living skills when persons are unable to do so due to disability issues, illness, injury or behavioral challenges.
14. Performs other duties as assigned by supervisor.

Qualifications
* Associate’s Degree preferred.  High school Diploma required.
* Three years of experience providing services to persons with developmental disabilities required.
* One year experience with medical support and AMAP certification required.
* Valid New York State driver’s license and an insured, registered motor vehicle.
* Physical capabilities to secure medical equipment and assist the physically challenged.
* Competence with basic computer skills.
* Ability to interact with a variety of medical professionals required.
* Ability to work autonomously required.
* Strong communication skills required.
* Excellent time management skills desired.
