JOB RESPONSIBILITIES

 

 

  1. Actively develops reliable evaluation tools and investigation practices.

     

  2. Maintains accurate and up to date knowledge of VESID, OPWDD,
    United Way, New York
    State Education, OSHA and all other governmental codes, rules, laws and regulations that pertain to LDA services.

     

  3. Implements system of audits and oversight to all agency services and components of operation to assure ongoing assessment of efficient, sound process.

     

  4. Provides continuous feedback and appropriate recommendations to programs on auditing tools outcomes and practices. 

     

  5. Completes the Quality Assistance Outcome and Compliance Report.

     

  6. Functions as a member of the program planning team when requested.

     

  7. Acts as referral agent and advocate representing the interests of individuals receiving services.

     

  8. Completes OPWDD 147 investigative process.  Presents investigation findings for administrative review and the Incident Review Committee.

     

  9. Reviews and completes follow up (if appropriate) for Internal Events for assigned programs.

     

  10. Participates in, and provides education and training to, individuals who receive services and staff.

     

  11. Develops and conducts quality improvement surveys for individuals receiving services and other appropriate parties as requested.

     

  12. Acts as agency liaison for OPWDD and other community organizations.

     

  13. Performs other duties as assigned by supervisor.

     


Qualifications:

QUALIFICATIONS

 

 

  • Bachelor’s Degree in human services or closely related field required.

     

  • Two years of OPWDD experience in a mid-level professional position required.

     

  • Experience planning and conducting confidential investigations and/or evaluating programs or processes in a professional manner, preferred.

     

  • Valid New York State Driver’s License.

     

  • Consistent access to a reliable vehicle with current registration and insurance.

     

  • Willingness to travel to various LDA programs and within the community.

     

  • Proficient computer skills in Microsoft Office.
